Before Your First Appointment
Please bring the following to your first visit:
- A form of photo identification
- Your health insurance card (if you have one)
- Social Security card (if applicable)
- Your current medications or a list of them
- Immunization records
- Proof of income (if you are a cash-pay patient)
- Having these items ready helps our team register your information correctly and begin your care without delay.
Forms to Complete at Check-In
At your appointment, you will complete a set of medical forms. These help us better understand your background, health priorities, and care needs.
You will be asked to complete:
- Patient Demographics Form
- Income Verification Form
- Medical Release Form
- Consent to Release Personal Health Information
- Staying Healthy Assessment
- Patient Health Questionnaire
- Adult TB Exposure Risk Assessment
- Appointment Guidelines for Medical Patients
- Social Determinants of Health Questionnaire
- These forms are provided in multiple languages so that we can accommodate patients from different cultural and linguistic backgrounds.
Patient Support Services
At CCHC, we want every patient to feel comfortable communicating with our team and accessing the services they need. Two important support services available to you.
Translation Services
- We offer translation assistance for patients who need extra help communicating during visits. Our staff includes bilingual team members, and we also provide interpreter support when needed. This service helps ensure that your questions and concerns are understood and addressed effectively.
Eligibility Assistance
- CCHC has team members who can help you understand insurance options and eligibility requirements. They can guide you through enrollment support, answer questions about coverage choices, and connect you with the right resources so you can make informed decisions about your care.

What to Expect on Your First Visit
When you arrive, our friendly front office staff will check you in, confirm your information, and review any forms you’ve completed. Your provider will then discuss your medical history with you, perform any necessary evaluations, and work with you to create a care plan tailored to your needs.
